Pediatric Oncosurgery is a specialized branch of pediatric surgery that deals with the surgical diagnosis, biopsy, staging, and treatment of solid tumors in infants, children, and adolescents. It is performed as part of a multidisciplinary team involving pediatric surgeons, pediatric oncologists, radiologists, pathologists, anesthesiasts, and radiation oncologists. The goals are complete tumor removal whenever possible while preserving organ function and minimizing long-term complications.

These conditions require urgent or emergency surgical intervention to save life or prevent irreversible complications:
Acute intestinal obstruction due to abdominal tumors (e.g., lymphoma, neuroblastoma)
Intussusception caused by a pathological lead point (e.g., lymphoma)
Tumor rupture with intra-abdominal hemorrhage (e.g., Wilms tumor, hepatoblastoma)
Massive intratumoral or postoperative bleeding
Gastrointestinal perforation secondary to tumor
Ovarian tumor with torsion
Testicular tumor with torsion (when diagnosis is uncertain)
Airway obstruction due to cervical or mediastinal tumors
Spinal cord compression requiring urgent decompression (selected cases)
Obstructive uropathy causing acute renal failure requiring urinary diversion
Tumor-related bowel ischemia or perforation
Placement of emergency vascular access for urgent chemotherapy when indicated

Most pediatric cancers are not surgical emergencies. Surgery is usually carefully planned after imaging, biopsy (when indicated), staging, and multidisciplinary discussion to achieve the best oncological and functional outcomes. Emergency surgery is reserved for life-threatening complications such as hemorrhage, obstruction, perforation, airway compromise, or organ-threatening compression.
